PILOT, CONTROLLERS BLAMED FOR AIR MISS Errors by controllers and pilots have been blamed for an air-miss in which two planes came within two thirds of a mile of each other in thick cloud.
The incident happened over Essex last year and involved a Japanese Airlines jumbo and a Gulfstream business jet.
An Air Accident Investigation Branch report says the jumbo pilot failed to descend to the level advised.
It also says air traffic controllers did not handle the flight properly.
